New Patient Registration

If you live within our practice area (marked on the map displayed in the waiting room) you are welcome to register with us and our reception staff will be happy to guide you through the procedure. When registering you must visit the surgery with the relevant documents required; 2 forms of ID one with photograph and one verifying your residing address.

To register please bring your medical card. This should be signed and your present address should be completed if this has changed. If you do not have a medical card, please ask reception for a GMS1 form.

All new patients over 5 years of age will be invited to make an appointment with the Practice Nurse for a general health check. This includes height, weight, blood pressure and a few general questions concerning your health, please bring a urine sample to this appointment and all current medication being used.

Change of Personal Details

If you change your name, address, post code or telephone number, please give full details to Reception.
